What to Bring Along
1. Fertility History Form
Before your first visit please complete our Fertility History Form and bring this form with you to your first appointment.
To download the form you will need Adobe Acrobat Reader.
If you are unable to access the form, please call us at 416-609-8868 and we will mail or fax you a copy.
2. Other documents and results of previous tests or treatments:
- Previous blood screenings
- Semen analysis reports
- Operative reports
- Results of previous uterine cavity and tubal patency testing
3. A Record of Your Periods
We will require a detailed record of the last 3-6 periods before your first visit. We need to know:
- The date each period started
- How many days there were between the first days of each period
